What is Chirayu Haryana Yojana?
Chirayu Haryana Yojana (also known as Ayushman Bharat - Chirayu Haryana) is a health insurance scheme by the Government of Haryana that provides cashless treatment of up to ₹5 lakh per family per year at empanelled hospitals. It is the state extension of the central Ayushman Bharat PM-JAY scheme.
The scheme covers families identified through the Parivar Pehchan Patra (PPP) database. It aims to provide quality healthcare to economically weaker families, covering hospitalization, surgeries, diagnostics, and medicines.
Who is Eligible?
- Residency: Must be a resident of Haryana with a verified Parivar Pehchan Patra
- Income: Families with annual income up to ₹1,80,000 (or as notified) are eligible
- PPP Verification: Family must be verified in the Parivar Pehchan Patra system
- No Other Insurance: Should not be covered under CGHS, ECHS, or similar central government health schemes
- SECC/Deprivation Criteria: Families identified under SECC data or state deprivation criteria

Check Eligibility & Download Card
- Visit chirayuharyana.gov.in
- Enter your Parivar Pehchan Patra (Family ID) number
- Check if your family is eligible
- If eligible, download or generate your Chirayu e-card
- Save or print the card for use at hospitals
At Empanelled Hospital
- Visit any Chirayu-empanelled hospital (government or private)
- Show your Aadhaar Card and Parivar Pehchan Patra
- Verification is done at the hospital's Ayushman desk
- Get cashless treatment — no out-of-pocket payment for covered procedures
If Not Auto-Enrolled
- Visit the nearest Common Service Centre (CSC) or Atal Seva Kendra
- Carry Parivar Pehchan Patra, Aadhaar, and income proof
- Request enrollment under Chirayu Haryana
- Biometric verification will be done
- Card will be generated after verification
