What is HP Sahara Yojana?
HP Sahara Yojana (also called Mukhyamantri Sahara Yojana) is a health assistance scheme by the Himachal Pradesh government. It provides ₹3,000 per month as financial help to patients suffering from serious and life-threatening diseases.
The scheme was announced in the HP Budget 2019-20 and officially launched on 11 September 2020. It is run by the Department of Health and Family Welfare, Himachal Pradesh through the HP Swasthya Bima Yojana Society (HPSBYS).
The main goal is to support poor families who face financial hardship due to prolonged treatment of chronic diseases. The monthly assistance helps patients meet medicine, travel, and daily care expenses during treatment.

The amount is transferred directly to the beneficiary's bank account every month. There is no limit on the number of months — assistance continues as long as the patient is under treatment and remains eligible.
Eligibility Criteria
To apply for HP Sahara Yojana, you must meet all these conditions:
- Resident of Himachal Pradesh — You must be a permanent resident of the state.
- Economically Weaker Section (EWS) — You should belong to a below poverty line (BPL) or EWS family.
- Annual family income below ₹4 lakh — Total income from all sources must not exceed ₹4,00,000 per year.
- Diagnosed with a listed disease — You must have a medical certificate or diagnosis report confirming your illness from a registered government hospital.
Documents Required
Keep the following documents ready before applying:
- Aadhaar Card — for identity verification
- Permanent Residence Certificate — proof that you are an HP resident
- Income Certificate — showing annual family income is below ₹4 lakh (issued by Tehsildar or SDM)
- BPL Ration Card (if available) — to prove economic status
- Medical Certificate / Diagnosis Report — from a government hospital confirming the disease
- Bank Account Passbook — with account number and IFSC (account must be linked to Aadhaar)
- Passport-size Photographs — recent photos of the patient
- Treatment Records — prescriptions and hospital records showing ongoing treatment
How to Apply for HP Sahara Yojana
Online Application
- Visit the official Sahara Yojana portal: sahara.hpsbys.in
- Click on "Apply Online" or go directly to the Aadhaar Verification page
- Enter your Aadhaar number and verify your identity
- Fill in the application form with personal details, disease information, and bank details
- Upload the required documents (medical certificate, income certificate, etc.)
- Submit the form and note down your application reference number
Offline Application
- Visit your nearest Community Health Centre (CHC) or District Hospital
- Ask for the Sahara Yojana Application Form
- Fill the form with all required details
- Attach self-attested copies of all required documents
- Submit the form at the hospital or the office of the Chief Medical Officer (CMO) of your district
ASHA workers and Anganwadi workers in your area can also help you fill the form and complete the process. The government provides an incentive of ₹200 to ASHA workers for helping identify and enrol beneficiaries.

Where to Get Treatment — Listed Hospitals
Treatment and diagnosis must be done at government hospitals recognised under the scheme. These include:
- IGMC Shimla (Indira Gandhi Medical College) — the main referral hospital
- Dr. RPGMC Tanda, Kangra (Dr. Rajendra Prasad Government Medical College)
- SLBSGMC Mandi (Shri Lal Bahadur Shastri Government Medical College)
- District Hospitals in all 12 districts of Himachal Pradesh
- Community Health Centres (CHCs) across the state
- Any other government hospital empanelled under HPSBYS
