What is Ayushman Bharat Health Card?

Ayushman Bharat Health Card (also called PMJAY card or Ayushman card) is issued under the Pradhan Mantri Jan Arogya Yojana (PM-JAY) scheme. It gives eligible families health insurance cover of ₹5 lakh per year for secondary and tertiary hospitalisation. The card is your proof of entitlement — you show it at any empanelled hospital to get cashless treatment.

There are two types:

  • Physical Ayushman Card — Printed PVC card distributed at Common Service Centres (CSCs) or camps
  • Ayushman e-Card — Digital version you can download and print from the Beneficiary Portal

Both are equally valid for availing cashless treatment at empanelled hospitals.

Who is Eligible for Ayushman Bharat Card?

Eligibility is based on the Socio-Economic Caste Census (SECC) 2011 data. The scheme covers:

  • Rural families identified under deprivation criteria (no adult earning member, kutcha house, SC/ST households, landless labourers, etc.)
  • Urban families in occupational categories (ragpickers, domestic workers, street vendors, construction workers, etc.)
  • Senior citizens aged 70+ — added under the expanded AB PM-JAY scheme

Note: There is no income limit specified. Eligibility is determined by the SECC database, not by income declaration.

How to Check Your Eligibility

Before downloading, confirm you are eligible:

  1. Visit the official portal: beneficiary.nha.gov.in
  2. Enter your mobile number
  3. You will receive an OTP — enter it to verify
  4. Search using your name, Aadhaar number, ration card number, or RSBY URN
  5. If your name appears in the list, you are eligible

How to Download Ayushman Bharat Card Online

Method 1: Download from Beneficiary Portal (beneficiary.nha.gov.in)

This is the primary method to download your Ayushman e-card:

  1. Go to beneficiary.nha.gov.in
  2. Click on "Login" or "Download Ayushman Card"
  3. Enter your registered mobile number
  4. Verify with the OTP sent to your phone
  5. After login, your family details and beneficiary list will appear
  6. Click on "Download Card" next to your name
  7. Complete Aadhaar-based eKYC (you will receive another OTP on your Aadhaar-linked mobile)
  8. Once verified, your Ayushman e-card (PDF) will be ready to download
  9. Save the PDF and take a printout for hospital visits

Method 2: Download via Common Service Centre (CSC)

If you cannot download online:

  1. Visit your nearest Common Service Centre (CSC)
  2. Carry your Aadhaar card and mobile phone (for OTP)
  3. The CSC operator will verify your identity and create your Ayushman card
  4. You will receive a printed PVC card on the spot
  5. A nominal service charge may apply at the CSC

Important: The Ayushman card itself is free of cost. The government does not charge any fee. Only CSC operators may charge a small printing/service fee.

Method 3: Get Card at Empanelled Hospital

  1. Visit any PMJAY-empanelled hospital
  2. Go to the Ayushman Mitra / PMJAY help desk
  3. Provide your Aadhaar card for eKYC verification
  4. The hospital staff will generate your Ayushman card on the spot

You can store your Ayushman card digitally in DigiLocker for easy access:

  1. Download the DigiLocker app or visit digilocker.gov.in
  2. Log in with your Aadhaar-linked mobile number
  3. Go to the "Search Documents" section
  4. Search for "National Health Authority" or "PMJAY" in the issuer list
  5. Select "Ayushman Bharat Health Account Card"
  6. Enter your PMJAY beneficiary ID or Aadhaar number
  7. The card will be fetched and stored in your DigiLocker account

How to Re-Download Ayushman Card If Lost

If you lose your physical card or need a fresh copy:

  3. Go to your profile and click "Download Card"
  4. Complete Aadhaar eKYC again if prompted
  5. Download and print the new copy

You can download the e-card unlimited times — there is no restriction. Alternatively, visit a CSC to get a new PVC card printed.

How to Check Empanelled Hospital List

To find hospitals where your Ayushman card is accepted:

  1. Visit hospitals.pmjay.gov.in
  2. Select your state and district
  3. Choose hospital type (public/private/both)
  4. Optionally filter by speciality (cardiology, orthopaedics, etc.)
  5. The list of empanelled hospitals with addresses and contact details will appear

How to Add Family Members to Ayushman Card

Ayushman Bharat covers the entire family as listed in the SECC database. To add a new family member:

  1. Visit the nearest CSC or empanelled hospital with Ayushman Mitra desk
  2. Carry the existing family member's Ayushman card
  3. Provide the new member's Aadhaar card for eKYC
  4. If the new member is listed in the SECC family data, their card will be generated
  5. For newborns, carry the birth certificate and parent's Ayushman card

Note: You cannot add members online. Family member addition requires physical verification at a CSC or hospital.
