  • You have a Tamil Nadu government-issued Smart Family Card (ration card) — the family card must show income below the threshold

The scheme auto-enrolls families based on the Family Card database. The eligibility check is done through your Smart Family Card (ration card) number.

Documents Required

  1. Smart Family Card (Tamil Nadu ration card) — mandatory proof
  2. Aadhaar Card — for identity verification
  3. Hospital admission documents (at the time of hospitalization)

How to Use the Scheme at a Hospital

Step 1 — Visit Empanelled Hospital

Go to any CMCHIS-empanelled hospital (government or private). Look for the "CM Health Insurance Scheme" board.

Step 2 — Go to the CMCHIS Help Desk

In the hospital, find the CMCHIS Help Desk (all empanelled hospitals must have one). Show:

  • Your Tamil Nadu Smart Family Card
  • Aadhaar card

Step 3 — Eligibility Verification

The help desk verifies your family's eligibility in real-time using the Family Card number.

Step 4 — Cashless Admission

If eligible, you are admitted cashlessly for covered procedures. No deposit required. The hospital claims from the government directly.

Frequently Asked Questions

1. I have a TN ration card but my income is ₹1 lakh/year. Am I eligible?

Current income threshold is ₹72,000/year for CMCHIS. If your income exceeds this, you may not be eligible. However, TN is expanding the scheme in phases — check cmchistn.com for current limits.

2. I need dialysis 3 times a week. Is it covered?

Yes. Kidney dialysis is covered under CMCHIS. Ongoing treatments may require periodic renewal authorizations at the empanelled hospital's help desk.

3. Can I use this scheme in another state (e.g., Apollo Chennai vs Apollo Delhi)?

CMCHIS is valid only at hospitals in Tamil Nadu. Hospitals outside TN are not in the network.

4. My family card has 6 members. Is there a per-person limit?

The ₹5 lakh limit is per family per year — shared across all members. If one member uses ₹3 lakh, the remaining family has ₹2 lakh for the year.

5. My husband died last year. Am I still covered under the family card?

The widow and dependent children continue to be eligible under the same family card. Contact your nearest CMCHIS facilitation centre if any updates are needed in the system.
