Why Does This Happen?
A UPI payment involves multiple systems communicating in real time โ your UPI app, your bank (remitter), NPCI's UPI switch, and the receiver's bank (beneficiary). The transaction can break at any point:
1. Server Timeout
Your bank processes the debit, but the response from NPCI or the beneficiary bank doesn't come back within the timeout window (usually 30 seconds). Your bank deducted the money, but the transaction is marked "Failed" because the confirmation loop never completed.
2. NPCI Switch Failure
NPCI acts as the middleman between banks. If NPCI's UPI switch experiences heavy load or a glitch, it may fail to relay the credit instruction to the beneficiary bank โ even though the debit already happened.
3. Beneficiary Bank Rejected Late
The receiver's bank received the credit request but rejected it after your bank already debited the amount. Common reasons: receiver's account is closed, frozen, or has restrictions.
4. Network Drop Mid-Transaction
If your internet connection drops after the debit instruction is sent but before the full transaction completes, your app shows "Failed" even though money was deducted.
5. Peak-Hour Overload
During salary days (1st, 7th of month), festivals, or month-end, UPI handles over 1 billion transactions daily. Server overload causes timeouts that lead to this exact problem.

How Auto-Reversal Works (NPCI Mandate)
As per NPCI and RBI guidelines, failed UPI transactions where money is debited must be automatically reversed:
- Timeline: Refund within T+5 business days (5 working days from the transaction date)
- RBI circular (September 2019): Banks must auto-reverse failed transactions within the prescribed TAT. For delays, the bank must pay โน100 per day as compensation to the customer
